At Country Connect Foundation, we believe that everyone deserves a safe place to call home. Our Homelessness Program works alongside individuals and families who are experiencing or at risk of homelessness, providing culturally safe, community-led support that responds to people’s unique needs and circumstances.

Operating across Nhulunbuy and Ramingining, our team provides more than just short-term relief. We focus on long-term solutions - offering case management, referrals, advocacy, crisis accommodation, outreach, and practical support to help people find stability, rebuild their lives, and reconnect with community.

We walk with people on their journey, with respect, dignity, and understanding - ensuring services are accessible, inclusive, and grounded in local knowledge and cultural understanding.

Services

  • Our short term accommodation provides a safe and stable environment for individuals experiencing or at risk of homelessness. We offer short term housing options while supporting residents to rebuild independence and work towards long-term housing solutions. Through close collaboration with housing providers, health services, and local agencies, we create tailored referral pathways that address the broader needs of each person we support.

  • Our flexible, person-centred case management is grounded in cultural understanding and shaped by the unique realities of remote and regional living. We work alongside individuals to identify their goals, navigate complex systems, and access the services and supports needed to move beyond short-term accommodation. This includes supporting people to build the skills and confidence required to maintain long-term housing, such as understanding tenancy responsibilities and skills development. Through strong partnerships with local service providers, we ensure referral pathways are well coordinated and that each person receives holistic, wraparound support tailored to their strengths, needs, and aspirations.

  • In response to the disproportionate housing crisis and severe overcrowding across the Northern Territory, our outreach service provides critical support to individuals who remain on waiting lists due to the limited availability of housing services. We continue to engage with people transitioning out of our short-term accommodation to ensure ongoing connection with services and reduce the risk of recurring homelessness. In collaboration with our valued partners, our outreach team also delivers essential brokerage assistance including food provisions, clothing, and other basic necessities - supporting individuals to maintain stability while they await access to safe and secure housing. Through proactive service delivery and coordinated referral pathways, we prioritise early intervention and holistic care.

  • In recognition of the disproportionate rates of homelessness experienced across the Northern Territory particularly among First Nations peoples, our short term accommodation is designed to respond to the needs of those most at risk. Support is prioritised for Aboriginal adults living with disability (diagnosed & undiagnosed), Aboriginal seniors aged 50 years and over, and Aboriginal adults experiencing home conflict - reflecting the urgent demand for culturally safe, appropriate services in remote and regional communities. This tailored approach ensures our efforts are directed where they are needed most, creating meaningful pathways out of homelessness and into long-term stability.
